The Open House Clinics will be held at the office of the Urban League of Philadelphia, 121 S. Broad Street (between Chestnut & Sansom Sts.), 9th Floor in the North American building on:

Wednesday, May 28th, 6pm - 8pm

Wednesday, June 25th, 6pm - 8pm

Individuals must bring proof of income, social security number, and information about child support, car insurance, childcare, work transportation expenses and legal guardian documentation with them to enroll.

The Urban League hopes to inform the community about CHIP’s comprehensive health care benefits, including routine doctor visits, immunizations, prescriptions, dental, eye care, hospital stays and more, available to all uninsured Pennsylvania children and teens who qualify for CHIP. To accomplish this goal, we are hosting Open House Clinics where we’ll assist parents in enrolling their children in this free/low-cost health insurance program. Please RSVP to: 215-985-3220 or info@urbanleaguephila.org .

ABOUT CHIP

CHIP offers affordable, comprehensive health care coverage to all Pennsylvania uninsured children and teens up to age 19. It offers coverage for services including doctors’ visits, hospital care, prescription drugs, vision, dental and home health care. Many families do not have to pay for CHIP. Premiums are affordable for all families because the premium payment is on a sliding scale based on the families’ income. There is no cost for families who qualify for free CHIP. More information on the CHIP program can be found by calling 1-800-986-KIDS or by visiting www.chipcoverspakids.com .
